Normally this happens due to a poorly soldered connection between the TruDrive and main board, the resistor on the main board right by the pad is loose, or potentially the TruDrive has been damaged due to static shock. Though it could be due to a soldering issue up on the LED iteslef. Is this on pad C1 or C2? What I would do is swap those two pads, and if the other die has the same issue, then it's not with the LED, but at the board side. Then I'd run a jumper from the board's pad that is bad (assuming it's C2, then #2) to #4 of the TruDrive, and hook the green die to C4.
If that works then you've shown the TruDrive was damaged. If you are not using c4 currently then you would be good after this; otherwise I can get you a new TruDrive for the cost of shipping, though of course that means it will be separated and require soldering with resistor clippings to keep it secure if you want it by the main board.
